Before you come to your first visit
To make the most of your first consultation, we ask you to submit the following materials prior to your visit either to our email:

All previous infertility medical records including lab work, any prior gynecological surgeries, your most recent pap smear, HSG films and your partner's semen analysis
We will provide you with an initial female and male patient history form. Please complete and then bring this with you for your consultation.
